Family Service of Chester County, a respected nonprofit human services agency serving Chester County, is seeking a Medical Case Manager to join its HIV/AIDS Prevention and Care Program at 1001 E Lincoln Hwy in Coatesville.
This is a full-time position (37.5 hours per week) available immediately for hire with a salary range of $45,000–$50,000 annually. People with a criminal record are encouraged to apply.

Position Summary
The Medical Case Manager will deliver comprehensive medical case management to individuals living with HIV/AIDS of diverse social, ethnic, and economic backgrounds. Key duties include:
- Assessing client needs
- Facilitating access to medical, social, and supportive services
- Making referrals to appropriate programs
- Maintaining accurate and timely documentation in client records
Requirements
- Bachelor’s degree in a human services–related field.
- Case management experience.
- Good communication and computer skills.
- Valid driver’s license and reliable transportation.
- Fluency in Spanish is a plus.
- Experience providing direct service to people living with HIV/AIDS is preferred.
- 2+Yrs Case Management experience required.
- Practicum or work experience in a health, welfare, or social service organization, or equivalent in supervised volunteer experience, is helpful.
- Candidate must have knowledge of HIV/AIDS disease and its treatment, be skilled in relating to minorities and clients of various backgrounds, orientations, and preferences.
- Must have organizational and data entry skills to meet the documentation and reporting requirements of the funding sources.
