Registered Nurse Assessment Coordinator

This position coordinates the completion, transmission, and monitoring of the MDS assessment process in accordance with State, Federal, and Barclay Friends standards; assesses residents, completes triggers, protocols, and care plans associated with the MDS for each resident in the skilled nursing unit; and acts as the facility-appointed Infection Preventionist.

The ideal candidate must be a graduate of an accredited school of nursing and have current licensure by the Commonwealth of Pennsylvania as a Registered Nurse., as well as a minimum of two years of experience as a professional nurse in long-term care. He/she must have strong clinical assessment and leadership skills and an understanding of PDPM. The person must demonstrate the ability to work as a team member and be detail-oriented, and will have effective problem-solving, writing, and interpersonal skills.

RN Supervisors

This position is responsible for managing staff, overseeing the care of the residents, and ensuring adherence to established policies and procedures. He/she provides hands-on care to residents as needed; however, the primarily role is to oversee the resident care provided by shift nurses. The person will ensure that the care the nurses provide to their residents is appropriate and comprehensive, and that staff-resident interactions are handled appropriately. The nurse supervisor will also interact directly with residents’ families to answer their questions and ease their concerns. He/she is responsible for ensuring staffing is at adequate levels to meet patient needs and address staffing concerns as they arise.

The ideal candidate will have graduated from an approved school of nursing and have current licensure by the Commonwealth of Pennsylvania as a Registered Nurse. He/she will be proficient in Microsoft Office and use of an electronic medical record system and will have a minimum of two years nursing experience. Supervisor experience is preferred.

Certified Nursing Assistants

These positions provide each assigned resident with routine daily nursing care and services, in accordance with the resident’s assessment and care plan and as directed by the supervising nurse.

The ideal candidates will have successfully completed a state-approved nursing assistant training program and possess certification.
