Early Intervention Service Coordinator

This position assesses client needs and develops personal service plans on an annual basis; oversees the development and implementation of the Individualized Family Service Plan; monitors client programs, per established early intervention requirements; and prepares monthly case management statistics.

The ideal candidate will possess a bachelor’s degree in social science or a related discipline; two years of human services work experience, including one year of experience providing counseling to children, families, or individuals with disabilities; and background and knowledge of social work and psychology.

ID Supports Coordinator

This position assesses the needs of clients with intellectual disabilities and develops proposed services and life management plans; ensures implementation and coordination of the plan; and makes necessary referrals and coordinates services as prescribed in the client’s proposed services plan.

The ideal candidate will possess a bachelor’s degree in a social welfare or social work-related discipline; two years of work experience in a human services field; experience with individuals who have developmental disabilities; and a minimum of one year of experience in social work.

Quality Improvement Specialist

This position assists with compliance activities related to fulfilling the requirements of the Administrative Entity Agreement with the ODP; develops effective training strategies for different stakeholder groups; and analyzes data and reports on status as needed.

The ideal candidate will possess a master’s degree in a human services discipline; five years of professional level-related work experience; working knowledge of applying quality management principles, strategies, and practices; and excellent verbal and written communication skills.
