The Phoenixville Community Health Foundation has opened nominations for the 2016 Champions of the Community Award which has been honoring members of the community who go above and beyond to help others since 2000.

The nominations are for the often overlooked stars of any age that have volunteered or given their skills in other ways to help out communities in the greater Phoenixville region. The foundation is looking for people members of the community feel should be recognized.

This could be someone who made a personal sacrifice to help improve the lives of others, has volunteered their time and served as a mentor to youth or a role model, or has simply been helpful to the people around them throughout their life.

Each of the chosen Champions will be honored with a $2,000 grant that will be donated in their name to the charity of their choice. Over the last 15 years, the award has honored sixty-eight unsung heroes as Community Champions. Nominees and awardees will be honored during an invitation only luncheon in late fall.

The deadline for submitting nominations is September 30th. To nominate someone, fill out the form on the Foundation’s website or contact Lynn Pike Hartman at LPHartman@pchf1.org  or on 610-917-9890 for any additional questions.
