PennMedicine210The Chester County Hospital is part of Penn Medicine, an organization dedicated to the health and well-being of the people in Chester County and surrounding areas. The hospital includes a 245-bed complex in West Chester and satellite locations in Exton, West Goshen, New Garden, Jennersville and Kennett Square. In 2013, Chester County Hospital joined the University of Pennsylvania Health System.

Monitor Surveillance Technician

This position is responsible for continuous observation of cardiac rhythms in the Central Monitoring Room. He/she will interpret cardiac rhythms and distinguish between normal and abnormal rhythms, and will report the information in an accurate, timely manner to nursing staff. The person will admit patient into system and verify that the patient is on the correct telemetry transmitter upon admission and every shift.

The candidate must be a high school graduate, and will have to successfully complete the CCH Dysrhythmia Course and MST orientation. Basic computer skills are necessary. He/she must have the ability to complete data entry on clinical documentation screens, and successfully complete on-the-job training for computer programs.

Case Manager

This position is responsible for the coordination of a multidisciplinary plan of care that ensures the delivery of quality patient care and facilitates discharge and expected outcomes, utilizing appropriate and cost effective strategies. He/she will perform and communicate clinical reviews and data to insurance payors, and will collaborate with healthcare team to streamline patient care. The person must also identify system barriers to cost-effective, quality care.

The candidate must be a registered nurse with a minimum of three years of acute clinical experience in the designated specialty. Past experience in a leadership role is preferred, as well as past utilization management and/or discharge-planning experience.

Nurse Practitioner / Physician’s Assistant

This position is a member of the hospitalist team and is responsible for co-management of adult patients under the hospital’s care. He/she will work collaboratively with the hospitalist physician to perform physical exams, collect and analyze data, formulate medical diagnoses, perform approved diagnostic and therapeutic procedures, order laboratory studies, prescribe appropriate interventions and medications, document patient history, and provide direct patient care and counseling.

The candidate must have completed an accredited Nurse Practitioner program and have a Master of Science Degree in Nursing, or have completed an accredited Physician Assistant program and have a Master of Medical Science.
