Full-Time Social Worker–Skilled Nursing and Short-Term Rehab
Dunwoody Village is is seeking a dynamic social worker within its Skilled Nursing and Short-Term Rehabilitation departments. This position does require working a weekend rotation along with our team of social workers.
This integral team member is responsible for completing biopsychosocial assessments and assuring comprehensive discharge plans for short-term residents. Additional duties of a Social Worker include:
- Maintaining responsibility for completing required documentation, including mood and cognitive assessments, and for providing excellent customer service.
- Leading care plan meetings and helping residents and their loved ones to understand their expected length of stay
- Implementing and discuss care planned goals and interventions towards safe discharge planning
- Acting as a liaison and point person for residents and responsible parties
- Assisting with insurance case management and reports to the Director of Social Service.
To qualify for this position, candidates must meet the following criteria:
- a Bachelor’s Degree in Social Work; candidates with a Master’s Degree are also encouraged to apply
- minimum of 2 years of relevant experience, preferably in a similar setting and population
- Working knowledge of Point Click Care and Microsoft Office – including Word and Outlook.
- Ability and keen desire to work effectively in a team-based setting.
- Willingness/desire to provide exemplary customer service to both internal and external customers

Full-Time Care Center Administrative Assistant
Dunwoody Village is seeking an experienced Care Center Administrative Assistant to join its team.
The Care Center Unit comprises 81 skilled nursing rooms and 81 personal care rooms. The Administrative Assistant is integral in ensuring that operations run smoothly and efficiently. Some of the key essential functions of the position include the following:
- Provides daily support to nurses on assigned units
- Preparing and compiling medical records
- Scheduling appointments and arranging transportation for our residents on our Skilled Nursing units.
- Scanning medical records and documents for electronic medical record
- Coordinates companions for residents when attending medical appointments.
- Assists our nursing staff with admission and discharge administration
- Serves as a backup receptionist as necessary
- Answers phones at the nursing station and directs calls
- Communicate with residents, families, and appropriate persons for efficient scheduling of appointments
- Maintains appropriate inventory of forms and supplies as well as nursing units
The ideal candidate is best described below:
- At least two years of broad clerical/administrative experience in a busy medical setting – preferably in a CCRC.
- Superb administrative skills with a detail-oriented approach to completing tasks.
- Working knowledge of Microsoft Office, including Word and Point/Click/Care (PCC), is strongly desired.
- Proficiency in medical terminology is strongly preferred

Pool Licensed Practical Nurses
Dunwoody Village is seeking qualified Licensed Practical Nurses (LPNs) for available PRN/Pool shifts:
- 7 AM-3:30 PM
- 3 PM-11:30 PM
- 11 PM-7:30 AM
Pool Nurses should be able to work some weekends, participate in our holiday rotation, and understand that they may be required to work on any of our various units.
Overall, LPNs would provide professional, supportive care to the residents of Dunwoody Village who reside in the Skilled, Personal Care, Memory Support, or Rehab areas. They would be responsible for evaluating and assessing the residents’ physical, mental, and psycho-social needs to determine if further professional intervention is appropriate.
Eligible candidates must meet the following requirements:
- Must be a PA licensed Registered Nurse (RN) with current CPR certification
- Minimum of 2 years of long-term care experience and excellent assessment skills
- Point Click Care experience
