Dunwoody at Home-Private Duty Companions
Dunwoody at Home is looking for experienced and reliable Home Health Aides/Private Duty Companions to join its team.
Interested candidates must be available to work Monday-Sunday for one of the following shifts: 7 AM-3 PM, 7:30 AM-11:30 AM, 12 PM-5 PM, 3 PM-11 PM, 6 PM-8 PM or 11 PM-7 AM. This position includes working a weekend and holiday rotation.
Duties of an aide include:
- Assisting with direct patient care procedures and related tasks as they pertain to safety, hygiene, nutrition, and socialization.
- Performing miscellaneous job-related duties as assigned. These may include the following: laundry, light housekeeping, shopping, transporting, and accompanying clients to various activities.
- Reporting any change in clients’ status to appropriate personnel. For example: change in mental status, change in appetite, and change in gait.
To qualify for this position, candidates must meet the following criteria:
- High school diploma or its equivalent
- At least one year of experience that is directly related to the duties and responsibilities specified above – preferably in a setting providing exemplary care for older adults. Although not required, PA Certified Nursing Assistants in good standing are welcome to apply.
- Knowledge of patient evaluation and triage procedures.
- Ability to maintain quality, safety, and infection control standards.
- Ability to lift and turn patients
- Knowledge of patient care charts and patient histories when applicable.
- Knowledge of related accreditation and certification requirements.
- Knowledge of specific patient care equipment is preferred.
Certification/Licensure/Registration:
- Successful completion of the Dunwoody at Home competency test or documentation of successfully completing a Competency Training Program or examination from another home care agency or registry, provided the break in employees’ roster or working status does not exceed twelve months.

Full-Time Social Worker
Dunwoody Village is seeking a dynamic full-time Social Worker for its Skilled Nursing and Short-Term Rehabilitation departments. This position requires working a weekend rotation along with our current team of social workers.
This integral team member is responsible for completing biopsychosocial assessments and assuring comprehensive discharge plans for short-term residents.
Additional duties of a Social Worker include but are not limited to:
- Maintaining responsibility for completing required documentation, including mood and cognitive assessments, and for providing excellent customer service.
- Leading care plan meetings and helping residents and their loved ones to understand their expected length of stay
- Implementing and discussing care-planned goals and interventions toward safe discharge planning
- Acting as a liaison and point person for residents and responsible parties
- Assisting with insurance case management and reports to the Director of Social Service.
To qualify for this position, candidates must meet the following criteria:
- A Bachelor’s Degree in Social Work; candidates with a Master’s Degree are also encouraged to apply
- Minimum of 2 years of relevant experience, preferably in a similar setting and population
- Working knowledge of Point Click Care and Microsoft Office – including Word and Outlook.
- Ability and keen desire to work effectively in a team-based setting.
- Willingness/desire to provide exemplary customer service to both internal and external customers

Certified Nursing Assistant
Dunwoody Village is currently seeking dedicated and caring Certified Nursing Assistants (CNAs) to fill open Pool/PRN positions for the following shifts:
- 7 AM-3 PM
- 3 PM-11 PM
- 11 PM-7 AM
Pool CNAs should be able to work some weekends, participate in a holiday rotation, and understand that they may be required to work on any of the various units.
Certified Nursing Assistant/CNA job duties include but are not limited to providing exceptional resident care according to a Plan of Care, assisting residents with personal care and hygiene, and providing a safe environment for residents and colleagues.
The position includes generous semi-annual bonuses, and a 403b retirement plan that includes a generous company match + a year-end discretionary contribution.
