Hospital-Based Social Worker
The Hospital-Based Social Worker will provide direct services to individuals affected by violence.
The social worker will assess client needs, develop and implement treatment plans, and provide ongoing evaluation of services. Reporting directly to the SVP, Community-Based Services, the successful candidate will have a strong commitment to public service.
This position is located in Pottstown.
Duties & Responsibilities
• Provide direct clinical services to clients, including assessments, counseling, and crisis intervention.
• Develop, implement, and evaluate individual treatment plans based on client needs.
• Participate in multidisciplinary team meetings and case discussions.
• Enter direct and indirect services into Simple Practice and documentation into the Tower Health EPIC system, as needed, within 2-3 business days from the service.
• Maintain additional thorough and timely grant documentation according to TLC and Tower Health regulatory standards.
• Participate in community outreach and violence prevention and intervention initiatives.
• Collect client information to begin case files and case plans, as well as conduct intake assessments.
• Coordinate services for clients, including counseling, medical care, legal assistance, and other necessary resources.
• Advocate for clients and act as a liaison with community providers and agencies.
• Foster and promote communication between Tower Health, TLC, and community resources.
• Provide crisis intervention and de-escalation as needed.
• Identify clients and family members in need of additional trauma support and provide prevention, referral, and treatment services.
• Provide home-based services to the families of clients as needed.
• Adhere to ethical and legal standards of social work practice.
• Participate in ongoing professional development and training.
• Perform other duties as assigned by the SVP, Community-Based Services.
Skills & Knowledge:
• Experience providing clinical services in violence intervention and trauma-informed care.
• Experience in case management and coordination of client care.
• Strong understanding of the psychosocial aspects of violence and trauma.
• Excellent oral and written communication skills.
• Ability to work with diverse groups and in challenging situations.
• Strong organizational and case management skills.
• Ability to work effectively on a multidisciplinary treatment team.
• Knowledge of community resources and networking ability.
• Proficiency in Microsoft Word, Excel, PowerPoint, and Outlook.
• Understanding of confidentiality and ethical guidelines in clinical services.
Education & Experience:
- Master’s degree in social work.
- Current license: either Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW).
- Minimum of 2 years of experience in violence intervention or trauma-informed care.

Board-Certified Behavior Analyst
The Board Certified Behavior Analyst (BCBA) will work with elementary, middle, and/or high school students at a Philadelphia-based charter school to (a) study the behavior of students and create plans to improve or change problematic behaviors; (b) work with students who have developmental disabilities, brain injuries or emotional control, social or behavioral issues; and/or (c) help students manage issues such as stress or anger in healthier ways or achieve personal behavior goals.
Duties & Responsibilities
- Meet with students and parents to observe and assess student behavior.
- Meet with families, teachers, or doctors to discuss the student’s treatment and progress and recommend ways to address behavioral issues.
- Develop individual plans to correct, maintain, or improve certain behaviors.
- Help clients set and meet behavior goals.
- Keep detailed notes of student meetings and progress.
- Use methods such as reinforcement and conditioning to correct or reduce aggressive, harmful, and anti-social behaviors and to encourage positive behaviors.
- Facilitate and assist in the development and identification of resources and support information for students and their families.
- Provide model teaching and other direct instructional supports, including but not limited to practicum.
- Develop and implement assessment tools; conduct functional assessments and analyses.
- Develop appropriate behavior strategies to teach appropriate behavior and reduce maladaptive behaviors.
- Conduct research and apply findings to their work.
Skills & Knowledge:
- Successful completion of internship and/or formal training in behavior analysis.
- Applied behavior management and analysis skills.
- Communication, organizational, and interpersonal relations skills.
- Ability to work with a variety of clients regarding age and functionality and with minimal direction.
Education & Experience:
Certification: Board Certified Behavior Analyst (BCBA)® Certification in Pennsylvania is required.

SAP School-Based Counselor
The School Counselor will work within a guidance program for students in middle and/or high school in Lansdale with a high degree of flexibility in serving the varied and diverse needs of the individual students.
The School Counselor will have a strong sense of compassion and commitment to helping people.
The School Counselor will be able to work in a team-driven environment with social services personnel, educators, and parents.
The School Counselor shall possess a high degree of sensitivity, warmth, and empathy while maintaining consistency with program expectations to encourage academic, social, and personal growth.
Essential Functions & Responsibilities:
• Work within middle and/or high school guidance program that provides students with a safe and non-judgmental way to voice their concerns or worries.
• Assist school administrators and educators with planning and carrying out school-related programs and events.
• Analyze student performance in the classroom to provide guidance and identify potential problems.
• Support the school IEP team and contribute to action items related to special education requirements.
• Learn students’ names, career objectives, and other details so as to better serve them.
• Help students develop academic plans in accordance with their skills, talents, and strengths.
• Work collaboratively with the multi-treatment team and other supportive staff
• Facilitate crisis intervention and prevention programs.
• Communicate with teachers, parents, and administrators on an ongoing basis about behavioral and academic problems.
• Provide counseling utilizing an individual and/or group format.
• Manage crises and ensure the safety of students at all times.
• Documentation must be completed on a daily basis.
• Attend all required meetings and support collaborative meetings with the school district and outside community agencies.
Skills:
• Excellent communication and organization skills.
• Ability to balance administrative and support services efficiently.
Education and Experience:
• Master’s degree in Social Work, Education, or School Counseling.
• SAP certification preferred.
