School-Based Creative Art Therapist

The School-Based Creative Art therapist will develop art-making processes to support students’ mental, emotional, and behavioral well-being within a school setting.

They will design and implement art therapy programs to help students cope with challenges, improve communication, and build self-esteem.

This role, located in West Lawn, Pennsylvania, often involves working with students facing issues like trauma, grief, learning disabilities, and behavioral problems.

Essential Functions & Responsibilities:
• Provide individual and group art-based interventions to address individual student needs and support group dynamics.
• Design and implement art therapy programs that align with school goals and student needs.
• Evaluate students’ progress and document observations and interventions.
• Collaborate with teachers, counselors, and other school staff to support student’s mental health and well-being.
• Promote the benefits of art therapy and advocate for mental health resources.
• Attend education/training on topics related to job duties

Hospital-Based Social Worker

The Hospital-Based Social Worker will provide direct services to individuals affected by violence.


The social worker will assess client needs, develop and implement treatment plans, and provide ongoing evaluation of services. Reporting directly to the SVP, Community-Based Services, the successful candidate will have a strong commitment to public service.

This position is located in Pottstown.

Duties & Responsibilities
• Provide direct clinical services to clients, including assessments, counseling, and crisis intervention.
• Develop, implement, and evaluate individual treatment plans based on client needs.
• Participate in multidisciplinary team meetings and case discussions.
• Enter direct and indirect services into Simple Practice and documentation into the Tower Health EPIC system, as needed, within 2-3 business days from the service.
• Maintain additional thorough and timely grant documentation according to TLC and Tower Health regulatory standards.
• Participate in community outreach and violence prevention and intervention initiatives.
• Collect client information to begin case files and case plans, as well as conduct intake assessments.
• Coordinate services for clients, including counseling, medical care, legal assistance, and other necessary resources.
• Advocate for clients and act as a liaison with community providers and agencies.
• Foster and promote communication between Tower Health, TLC, and community resources.
• Provide crisis intervention and de-escalation as needed.
• Identify clients and family members in need of additional trauma support and provide prevention, referral, and treatment services.
• Provide home-based services to the families of clients as needed.
• Adhere to ethical and legal standards of social work practice.
• Participate in ongoing professional development and training.

Special Education Teacher

The Special Education Teacher position, which includes a generous sign-on bonus, works with students who have a wide range of socio-emotional and mental health needs and/or learning disabilities.

The Special Education Teacher develops and provides specialized instruction to meet the unique needs of students, evaluates and assesses student progress against instructional objectives, follows PA-mandated due process procedures, and functions as IEP Manager to assigned students.

The Special Education Teacher, operating out of West Lawn, PA, must be able to work in a team environment with other educators, special education staff, counselors, and social workers. The Special Education Teacher should possess a high degree of authenticity, kindness, and empathy while effectively maintaining an environment conducive to academic and socioemotional growth.

Duties & Responsibilities
• Work with students in grades PreK-4 who have a wide variety of socioemotional and mental health needs and/or learning disabilities.
• Assess students’ skills to determine their needs and to develop teaching plans.
• Plan, organize, and assign activities that are specific to each student’s abilities.
• Teach and mentor students as a class, in small groups, and one-on-one..
• Assess student progress and determine the need for additional reinforcement or adjustments to instructional techniques.
• Establish and maintain student control and discipline in the classroom, on school premises, or during school activities.
• Employ and implement a classroom structure to encourage student responsibility, cooperation, and mutual respect consistent with school policies and procedures.
• Provide consultation to classroom teachers regarding classroom adaptations, instructional modifications, adaptive equipment, behavior modification plans, and other similar instructional interventions to meet the needs of students.
• Develop and implement Individualized Educational Program (IEP) plans for students to include present levels of educational performance, special education needs, instructional goals and objectives, and the special education and related services required to meet those goals.
• Implement IEPs, assess students’ performance, and track their progress.
• Update IEPs throughout the school year to reflect students’ progress and goals.
• Discuss student’s progress with parents, teachers, counselors, and administrators.
• Employ various teaching techniques, methods, and principles of learning to enable students to meet their IEP goals.
• Provide research-based specialized instruction to address the instructional goals and objectives contained within each student’s IEP.
• Coordinate the delivery of special education services in each student’s IEP.
• Supervise and mentor other teachers who work with special education students.
• Collaborate with teachers, counselors, and social workers to accommodate students’ needs.
• Meet with parents, school administrators, and counselors to discuss updates and changes to the IEPs.
• Schedule team meetings and work cooperatively with child study team members and others to develop instructional goals and strategies.

• Complete all district and state reporting requirements.

Director of Scholar and Wellness

The Director of Student Wellness will lead the expansion of a robust school-based mental health services model, including developing, implementing, and overseeing comprehensive mental health and wellness programs within Breakthrough Public Schools.

Under the leadership of the Managing Director of Culture, the Director of Student Wellness will support the implementation of a federal grant to expand the school’s school-based mental health program.

The role offers the opportunity to develop innovative school-based mental health support and external partnership models rooted in student, family, and staff needs.

This role, located in Cleveland, Ohio, requires a strong background in mental health, leadership, program development, and collaboration.

Duties and Responsibilities:

  • Create short-term and long-term plans to enhance mental health support, de-stigmatize mental health issues, and prioritize overall well-being.
  • Develop and implement crisis intervention protocols and offer guidance and immediate support to students, staff, and the school community during times of crisis or emotional distress.
  • Develop, implement, and manage mental health and wellness programs, initiatives, and resources tailored to students’ needs.
  • Implement multi-tiered behavior support with fidelity in collaboration with principals, school teams, and social workers.
  • Collect, analyze, and report program effectiveness, engagement, and outcomes data.

Skills and Knowledge:

  • Lead a team of social workers and counselors by providing coaching and development, ensuring a cohesive and effective approach to mental health support.

  • Hold or have the ability to gain a Pupil Services License (OH).
