Headquartered in Audubon, TLC operates across multiple states, providing K-12 alternative education, comprehensive mental and behavioral health services, coaching, counseling, and trauma-informed training.
TLC collaborates with school districts, universities, hospitals, police departments, and nonprofit organizations, offering consultation services to secure federal and state grant funding.
Here are a few of the staff positions currently available at The Lincoln Center for Family and Youth:
Full-time Hospital-Based Social Worker
The Hospital-Based Social Worker will provide direct services to individuals affected by violence.
The social worker will assess client needs, develop and implement treatment plans, and provide ongoing evaluation of services.
Reporting directly to the SVP, Community-Based Services, the successful candidate will have a strong commitment to public service.
Duties & Responsibilities
- Provide direct clinical services to clients, including assessments, counseling, and crisis intervention.
- Develop, implement, and evaluate individual treatment plans based on client needs.
- Participate in multidisciplinary team meetings and case discussions.
- Enter direct and indirect services into Simple Practice and documentation into Tower Health EPIC system, as needed, within 2-3 business days from the service.
- Maintain additional thorough and timely grant documentation according to TLC and Tower Health regulatory standards.
- Participate in community outreach and violence prevention and intervention initiatives.
- Collect client information to begin case files and case plans, as well as conduct intake assessments.
- Coordinate services for clients, including counseling, medical care, legal assistance, and other necessary resources.
- Advocate for clients and act as a liaison with community providers and agencies.
- Foster and promote communication between Tower Health, TLC, and community resources.
- Provide crisis intervention and de-escalation as needed.
- Identify clients and family members in need of additional trauma support and provide prevention, referral, and treatment services.
- Provide home-based services to the families of clients as needed.
- Adhere to ethical and legal standards of social work practice.
- Participant in ongoing professional development and training.
- Perform other duties as assigned by the SVP, Community-Based Service.
Skills & Knowledge
- Experience providing clinical services in violence intervention and trauma-informed care
- Experience in case management and coordination of client care
- Strong understanding of the psychosocial aspects of violence and trauma
- Excellent oral and written communication skills
- Ability to work with diverse groups and in challenging situations
- Strong organizational and case management skills

Education and Experience
- Master’s degree in social work.
- Current license: either Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW).
- Minimum of 2 years of experience in violence intervention or trauma-informed care.
This is a full-time, 40 hours per week, position. Hours to include 11 AM to 7 PM Monday through Friday.

Full-time Licensed School-Based Counselor/Social Worker
The School-based Counselor/ Social Worker provides early intervention therapy and counseling services in a Technical School.
They also provide case management and mental health support services at the school and in the homes of at-risk students.
The counselor/social worker will provide consultation services to school personnel, liaison support to community agencies, and training programs for school and community populations.
Services to students include:
- Identify students in need of trauma support and provide prevention, screening, referral and treatment services to students potentially in need of service.
- Provide trauma-informed therapy and counseling interventions (group, individual and/or family) for at-risk students.
- Provide mentoring and other intervention models to children and their families who have experienced trauma or are at risk of experiencing trauma, including those who are low-income, homeless, in foster care, involved in the criminal justice system, unemployed, experiencing a mental illness or substance abuse disorder or not enrolled in or at risk of dropping out of an educational institution.
- Provide case management for students and families requiring multiple resources.
- Provide quality trauma support and behavioral health care services.
- Provide home-based services to the families of students who have experienced or are at risk of experiencing trauma, including those who are low-income, homeless, involved in the child welfare system or involved in the juvenile justice system.
Services to school personnel include:
- Assist teachers with behavior management strategies.
- Provide staff with essential information to better understand factors (cultural, societal, economic, familial, health, etc.) affecting a student’s performance and behavior.
- Train school staff on trauma-informed approaches to education to develop safe, stable and nurturing learning environments that prevent and mitigate the effects of trauma.
Services to districts include:
- Provide consultation regarding school law and school policy including IDEA and Section 504.
- Assist in developing positive behavioral intervention strategies.
- Assist in developing and implementing educational programs, including programs on safety and bullying.
- Develop alternative programs for drop-out, truancy, delinquency, etc.
Services to parent/families
- Work with parents to facilitate their support in their children’s school adjustment.
- Alleviate family stress to enable the child to function more effectively in school & community.
- Assist parents in accessing and utilizing school and community resources.
- Work with those problems in a child’s living situation that affect the child’s adjustment in school (home, school, and community).
School Community Liaison:
- Obtain and coordinate community resources to meet students’ needs.
- Provide health services and intervention strategies by coordinating the services provided by eligible applicants and coordinated care organizations, public health entities, nonprofit youth service providers and community-based organizations.
- Foster and promote communication between the school entity, community and law enforcement.
- Increase access to quality trauma-informed support services and behavioral health care by linking the community with local trauma support and behavioral health systems.
- Help school districts receive adequate support from social and mental health agencies.
- Advocate for new and improved community/school service to meet the needs of students and families.
- Mobilize family, school, and community resources to enable the child to learn as effectively as possible in his or her educational program.

Full-Time Licensed Behavior Specialist
This position specializes in behavior change. The function of this role is to provide direct assistance to educational staff, administrators, agency personnel, and parents in the areas of behavior change and addressing challenging behaviors of students.
The Behavior Consultant provides individual and group trainings, conducts and supports functional behavior assessments and positive behavior support plans, advises administrators and staff regarding best practices and regulations, works as a collaborative consultant with school and agency personnel, and provides expertise regarding behavior modification in complex cases.
The Behavior Consultant provides services and documents that are compliant with federal, state, and local regulations. Also, services and documents are based on best practices, research-validated methods, and PDE-supported procedures and materials.
Skills and Knowledge
- Provide consultation to staff through observations, functional behavior assessment, behavior modification plan development, consultations, and recommendations related to the needs of individual students, utilizing best practice, research validation methodologies, and PDE-endorsed practices.
- Conduct functional behavioral assessments for students in the school, home, or community setting.
- Create individualized positive behavioral support plans to address the target behaviors.
- Train staff, parents, and students on the implementation of the plan. Monitor treatment integrity and provide feedback when needed.
- Develop and oversee ongoing behavior data collection and present summarized data when needed.
- Assist staff in maintaining the level of expertise within their programs regarding the proper methods of de-escalating challenging and complex behaviors presented by students.
- Provide direct intervention support to students both proactively to teach new skills and reactively to facilitate the de-escalation of the student.
- Provide training, staff development, and informational support in all aspects of behavior change with challenging behaviors to LEA staff, parents, students, and agency personnel.
- Maintain professional and legally compliant documentation of services and student-related paperwork.
- Stay knowledgeable in best practices and research-validated practices.
Education and Experience
- Master’s Degree required.
- Licensed Behavior Specialist Consultant in Pennsylvania required.
- Experience working with children and adolescents with severe and complex behavior challenges in a school, community, or residential setting is required.
