Full Time Social Worker

Dunwoody Village seeks a dynamic Full-Time Social Worker within its Skilled Nursing and Short-Term Rehabilitation departments. This position does require working a weekend rotation along with a team of social workers and Social Services Manager. 

This integral team member is responsible for completing biopsychosocial assessments and assuring comprehensive discharge plans for short-term residents. Additional duties of a Social Worker include:

• Maintaining responsibility for completing required documentation, including mood and cognitive assessments, and for providing excellent customer service.
• Leading care plan meetings and helping residents and their loved ones to understand their expected length of stay
• Implementing and discussing care planned goals and interventions towards safe discharge planning
• Acting as a liaison and point person for residents and responsible parties
• Assisting with insurance case management and reports to the Director of Social Service.

To qualify for this position, candidates must meet the following criteria:

• A Bachelor’s Degree in Social Work; candidates with a Master’s Degree are also encouraged to apply
• A minimum of 2 years of relevant experience, preferably in a similar setting and population
• Working knowledge of Point Click Care and Microsoft Office – including Word and Outlook.
• Ability and keen desire to work effectively in a team-based setting.
• Willingness/desire to provide exemplary customer service to both internal and external customers

Full Time Registered Nurse

Dunwoody Village is seeking dedicated and caring Registered Nurses (RNs) to join its team for a Full Time 11PM-7:30 AM position which includes working a weekend and holiday rotation.

Those hired will receive a $20,000 sign on bonus paid during their first year of employment. 

Overall, RNs provide professional supportive care to the residents of Dunwoody Village who reside in the Skilled, Personal Care, Memory Support, or Rehab areas. They will be responsible for the evaluation and assessment of the physical, mental, and psycho-social needs of the resident to determine if further professional intervention is appropriate.

Eligible candidates must meet the following requirements:

• Must be a PA licensed Registered Nurse (RN) with current CPR certification
• Minimum of 2 years of long-term care experience and excellent assessment skills
• Point Click Care experience

Certified Nursing Assistant

Dunwoody Village is seeking a qualified Certified Nursing Assistants (CNAs) for available PRN/Pool shifts:

• 7AM-3PM
• 3PM-11PM
• 11PM-7AM

The Pool staff should be able to work some weekends, participate in our holiday rotation, and understand that they may be required to work on any of our various units.

Certified Nursing Assistant/CNA job duties include providing exceptional resident care according to Plan of Care, assist residents with personal care and hygiene and provide a safe environment for our residents and colleague

Eligible candidates must meet the following requirements:

• Must be a PA licensed CNA with current CPR certification
• Minimum of 2 years of long-term care experience and excellent assessment skills
• Point Click Care experience
