La Comunidad Hispana is a diverse, bilingual Federally Qualified Health Center in southern Chester County. LCH provides integrated, Person-Centered services through its health center, social assistance, and education classes and job placement. Its mission is to help low-income residents stay healthy, build strong families, and lead productive lives by providing high-quality, culturally welcoming services. Behavioral Health Consultant

This position provides counseling and intervention services to help clients reach their full human potential. Duties include assisting clients to remove barriers to health and wellness. He/she will collaborate with members of the health care team, the community service team, and the client/family to assure clients’ psychosocial needs are addressed.

The ideal candidate will be bilingual and possess a Master’s Degree in social work, or a related field, and have a minimum of two years of clinical experience. Experience with motivational interviewing, cognitive behavioral therapy, behavioral activation, and other short-term treatment approaches is preferred.

Registered Nurse

This position will provide chronic care management to high-risk patients, perform triage (phone and in-person), and assist in the care of chronic and primary care patients. He/she will provide education to all patients having chronic illness to further patient understanding of condition and treatment choices, with an emphasis on patients with diabetes, hypertension, asthma, hyperlipidemia, and history of ischemic vascular disease.

The ideal candidate will be a graduate from an accredited School of Nursing with a Bachelor’s of Nursing Degree. Public health/community health experience is preferred. Experience working with a second culture is also preferred.

Certified Registered Nurse Practitioner

This position will assure the provision of quality primary care services to Chester County families.  Services of the Family Nurse Practitioner at LCH include the following: physical examinations and immunizations, chronic disease management, acute episodic care for infants, children, adolescents, adults, and geriatric clients.

The ideal candidate will be a graduate from an accredited School of Nursing with a Master’s Degree in Advance Practice Nursing. He/she should be credentialed or eligible for credentialing under Medicare, Medicaid, and other commercial insurance. The person should be eligible for prescriptive privileges in Pennsylvania.
